I used Smartripperto extract the stuff from the PAL disk to my computer,
TmpgEnc to re-encode as an NTSC mpeg, and then used my dvd authoring program
to burn it to a DVD.  I didn't bother about chapters and all that stuff so
basically I got a dvd which plays from start to finish with no menu or
option - but it did the trick for me - I used the info contained in the
http://www.vcdhelp.com/ to do all of it.

the procedure for dvd would be exactly the same with resolution and
bitrate being the only real difference.

so instead of 352x240@1150kbps use 720x480 vbr <8mbs
tmpgenc will do the job.
